Each claim is different, let us guide you through your claim:

STEP 1

Fill in the relevant claim forms and mail all original copies of relevant documents to Liberty within 90 days from the date of consultation.

STEP 2

Upon receiving all original supporting documents, the claim will be processed an estimated 10 working days.

STEP 3

Claim payment will be processed to the mode of payment that you have selected.

What is the grace period to submit a claim?

All claims must be lodged with all relevant documents within 90 days from the date of consultation.
What are the supporting documents required?

Original official receipt(s) showing the name of the patient (must be identical with ID Card), diagnosis, date of consultation, itemised charges with the attending physician’s chop, and signature.

Please note the above list of documents is not exhaustive. Other documents may be requested if necessary.
Can I email Liberty the scanned documents instead?

No. The documents can be scanned and emailed to us, however, we will need the original copies to process the claim.
How much can I claim from my policy?

This depends on the benefit of entitlement in your policy. Please refer to the Schedule of Benefits.
What is a deductible/co-insurance?

The portion of costs for which the insured person/claimant is responsible. The deductible/co-insurance is applicable for each and every diagnosed medical condition for which a claim is made within any one policy year.
How can I get back my claim receipt?

Please state your request on claim form during submission. A certified true copy of the receipt(s) will be returned for the claims with the remaining balance.
What if we have questions about the claim settlement?
